HIPAA Notice of Privacy Practices

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

1. Our Responsibilities

Happy Drip IV & Wellness ("we," "us," or "our") is required by law to maintain the privacy of your Protected Health Information (PHI) and to provide you with this Notice of our legal duties and privacy practices. We will abide by the terms of this Notice.

2. Uses and Disclosures of Your PHI

We may use and disclose your PHI for the following purposes without your authorization:

Treatment

To provide, coordinate, and manage your IV therapy, weight loss programs, and other wellness services.

Payment

To bill and obtain payment from you, your insurance company, or another third party.

Health Care Operations

For quality assessment, staff training, and general business planning.

Other Permitted Uses

As required by law, for public health activities, health oversight audits, and to prevent serious threats to health or safety.

Uses and disclosures for marketing purposes or the sale of your PHI require your written authorization.

3. Your Rights

You have the right to:

• Inspect and copy your PHI.
• Request an amendment to your PHI if you believe it is incorrect.
• Receive an accounting of certain disclosures we have made of your PHI.
• Request restrictions on certain uses and disclosures of your information.
• Request confidential communications (e.g., by alternative means or at an alternative location).
• Receive a paper copy of this Notice upon request.

4. Complaints and Contact Information

If you believe your privacy rights have been violated, you may file a complaint with us or with the Secretary of the U.S. Department of Health and Human Services. There will be no retaliation for filing a complaint.

5. Changes to This Notice

We reserve the right to change the terms of this Notice. Any changes will be effective for all PHI we maintain. The revised Notice will be posted on our website and available at our facility.
